Mystery Ranch 2 Day Assault Pack, maybe with an aftermarket hip belt. It's a little big for under the seat in front of me but goes fine in the overhead. I'm skeptical anything in your size range is actually a good personal item though.

Help with 2 backpacks!
Laptop 2 - love my Mystery Ranch 2 Day Assault Pack. I'm not sure about 1. I don't do a second monitor or external keyboard. I don't think I've nailed my business travel setup even without that. The 2DAP is reasonably good but I hate putting it under the seat in front of me, since it's a bit bulky.
